United Healthcare, Golden Rule, and Evercare follow United Healthcare’s medical necessity polices for the Allergy and Hepatitis tests. If the medical necessity requirements are not met they will deny the claim and could possibly leave the balance as the patient’s responsibility. You can find these policies at http://www.clpct.com/professionals/testingeducation/rast/www.unitedhealthcareonline.com under Tools and Resources > policies and protocols > reimbursement policies. Also following are the links for the covered diagnoses lists.
