About CLP

Health Plans: Point of Service or HRA

Participation: All full-time and eligible part-time employees (30 hours or more per week) and eligible dependents.

Eligibility Date: First day of the month after date of hire, at annual open enrollment or following a qualifying event.

Cost: Both Employee and Company shares Cost.

Provider: POS Providers: Aetna or ConnectiCare. HRA Provider: Aetna

Highlights: No referral necessary for participating specialists. POS: Co-pay applies to in-network, non-specialists and specialists; 10% co-insurance applies on remaining bill. Preventative Care paid at 100%. Deductible applies to out-of-network care. HRA: Deductible applies to in and out-of-network care. After deductible is satisfied, 10% co-insurance applies to remaining balance of bill. For both plans, retail co-pay per prescription for a 30 day supply: Generic: $10 co-pay, Formulary Brand Name: $20 co-pay, Brand Name Non-Formulary; $40 co-pay.

Dental

Participation: All full-time and eligible part-time employees (30 hours or more per week) and eligible dependents.

Eligibility Date: First day of the month after date of hire, at annual open enrollment or following a qualifying event.

Cost: Both Employee and Company shares Cost.

Highlights: Provider is Delta Dental. Calendar year deductible of $50.00; dental service payable at 100% for preventive care and diagnostic services; 85% for remaining basic care; 50% for crowns and prosthodontics (after deductible); calendar year maximum of $1,000; orthodontia (adults & children) co-insurance 50%, lifetime maximum $1,000.

Short Term Disability

Participation: AFLAC plan: All full-time and eligible part-time employees (budgeted to work 30 hours or more per week). UNUM plan: All full-time and eligible part-time employees (budgeted to work 20 hours or more per week)

Eligibility Date: AFLAC plan: First day of the month after date of hire or date of election. UNUM plan: First day of the month after date of hire or at time of annual open enrollment.

Cost: AFLAC plan: Paid by Employee. UNUM plan: Paid by Company.

Providers: AFLAC and UNUM

AFLAC Highlights: 60% of basic earnings for up to 3 months of disability. (Subject to applicable tax)

UNUM Highlights: 70% of basic earnings for up to 12 weeks of disability, after 1 week of PTO time used. Maximum of $1,750 per week. (Subject to applicable tax)

Long Term Disability

Participation: All full-time employees and eligible part-time employees (budgeted to work 30 hrs or more per week)

Eligibility Date: First day of the month after date of hire.

Cost: Paid by Company

Highlights: LTD plan provides up to 60% of basic earnings after 90 days of disability. (Subject to applicable tax)

Group Term Life Insurance and AD&D

GTL plan death benefit pays 1x basic earnings, rounded up to the nearest thousand.

Double amount paid for accidental death & dismemberment.

Supplemental Life Insurance

Participation: All full-time and eligible part-time employees (30 hours or more per week).

Eligibility Date: First day of the month after date of hire.

Cost:Paid by Employee.

Provider: UNUM

Coverage: Two-Five times basic earnings in additional insurance.

Savings & Retirement Plan 401(k)

Participation: All full-time and part-time employees.

Eligibility: Employees must be 21 years of age.

Eligibility Date: First day of the month after date of hire and during quarterly open enrollment.

Cost: Paid by Employee

Provider: MassMutual

Match: 50% company match up to 6% deferral.

Highlights: The plan allows employees to contribute up to 50% or $16,500/year of their annual salary, whichever is less. Employees 50 years or older may contribute up to an additional $5,500 for a maximum of $22,000/year. There is a 3- year cliff vesting schedule for the company’s contribution.

Employee Assistance Program

Highlights: Available to all employees and members of their household. Confidential counseling services and referrals are available to employees and their family members. A toll-free number is available 24 hrs/day, 7 days per week to connect you with a professional counselor.

Flexible Spending Accounts

Participation: All full-time and part-time employees (20 hours or more) at date of hire.

Eligibility Date: First day of the month after date of hire and at annual open enrollment.

Cost: Paid by Employee (pre-tax).

Coverage: Provider is ADP. This program enables employees to contribute pre-tax dollars to help pay for dependent/elder care or health and dental care expenses not covered by insurance. An employee may elect up to $5,000 for the health care account and up to $5,000 for the dependent care account.

Dollars contributed to the Flexible Spending Account come out of gross earnings before tax. Certain IRS restrictions apply.

Paid Time Off (PTO)

Participation: Eligible full-time and part-time employees.

Eligibility Date: Accrual begins following 3-months from date of hire.

Cost: Paid by Company.

Highlights: Full-time, non-exempt employees who have worked less than 5 years, accrue 2.64 hours per week; after 5 years, 3.24 hours per week; after 10 years, 3.84 hours per week; after 20 years, 4 hours per week. Full-time, exempt employees who have worked less than 5 years, accrue 3.08 hours per week; after 5 years, 3.68 hours per week; after 10 years, 4.32hours per week; after 20 years, 4.4 hours per week.

Maximum accrual: 272 hours.

Credit Union

Available to all employees through the Hartford Healthcare Credit Union.

Holidays

Participation: All full-time and part-time employees.

Eligibility Date: Immediately

Cost: Paid by Company

Paid Holidays: New Year’s Day; Memorial Day; Independence Day; Labor Day; Thanksgiving Day; Christmas Day.

Tuition Reimbursement

Participation: All full-time and part-time employees (20 hours or more.)

Eligibility Date: Immediately

Highlights: Full time employees may be reimbursed up to a maximum of $3,000 and part-time employees are eligible to be reimbursed up to maximum of $1,500.
